Instructions for filling of feedback form
1. All questions are mandatory.
2. Every answer is to be supported by evidence and to be uploaded in Browse section shown at the bottom of each feedback form in PDF (.PDF) format only.
3. The supported document of each evidence should be properly numbered mentioning the question number on the top of the evidence document for which the same is attached.
4. All the supportive documents of evidence should be uploaded in one go in the Browse Section.
